Event Sign-up Forms (Scroll Down and Click on the event name to download form)

The regular tour Event fees will apply to ALL players. Non-tour players are required to pay the same event fees to play in the same weekend events. (Again, this year with assistance from the Host Course PGA Pro, we intend to post daily tee times and pairings for each event. Just click on the "Tee Times" button on the website Home Page.)

If you choose to pay by Credit Card using Google Checkout, click on the Google Buy Now/Checkout Icon above. Next, in the drop Down menu, select the appropriate fee for the tournament you intend to play in. Initially, and one time only, you will need to open an account with Google. Both you and QCAT (Dan Budde) will recieve a confirmation of payment by email. This will serve as your event registration and there is no need to fill out the forms below with the exception of the Quad City Amateur Event at Emeis because this event will not accept the Google Checkout Credit process.

If you intend to pay by personal check, just click on the event name below to download and print the event registration form and mail it with your check to the address provided thereon. With the exception of the Quad City Amateur event make all checks payable to "Quad City Amateur Tour".

Glynns Creek Amateur (May 15 - 16) - ($92)

QC Amateur (Emeis GC) (May 27-28-29) - ($120 for 3-Day Championship Flight or $95 for 2-Day All Other Flights make the check out to "Quad-City Amateur" and send to "Quad-City Amateur, 15505 - 108th Avenue Place, Davenport, IA 52804.)
